By providing information through the ACP Impact Stories Form, I hereby grant permission to EducationSuperHighway, a California nonprofit public benefit corporation, having its principal place of business at 6 Presidio Terrace, San Francisco, CA 94118 (“ESH”) and its representatives to use the following personal information:

• My name
• The location of my residence (city or town, and state)
• The name of my building, community, school, school district, organization, or government agency or entity
• Age
• Any narrative account of my experience
  
  

in ways including, but not limited to, news releases, print and media pitches, marketing materials (including, but not limited to, newsletters, blog posts, brochures, presentations, and reports), reports to funders, websites, or social media posts or ads, for purposes of information, education, and marketing of ESH and its programs, resources, services, and partners.

I further grant ESH and its representatives and licensees the right to reproduce, use, exhibit, display, broadcast and distribute and create derivative works of these narrative accounts (alone or together with other materials) for any purposes that ESH deems fit to advance ESH’s mission. ESH representatives include, but are not limited to, ESH partner organizations who facilitate collection of the narrative accounts. I acknowledge that ESH owns all rights to the narrative accounts in any medium. I waive any right to inspect or approve the use of the narrative accounts. I further waive any right to compensation arising from or related to the use of the narrative accounts or materials. I further grant ESH and its representatives the right to contact me to discuss the information provided to ESH pursuant to this General Consent and Release: Virtual Submissions.

I hereby release and forever discharge ESH, its governing bodies, officers, agents, employees, and representatives acting on behalf of ESH from and against any claims, damages or liability arising from or related to the use of the narrative accounts or materials, including but not limited to claims of defamation, invasion of privacy, rights of publicity or copyright infringement.

I also hereby acknowledge that I understand that:

• My right to services from ESH will not be affected if I do not agree to this General Consent and Release: Virtual Submissions.
• I will be provided with a copy of this General Consent and Release: Virtual Submissions.
• Materials produced relying on this General Consent and Release: Virtual Submissions may remain in use or circulation for an unknown period of time.
• Once my information is disclosed as described in this General Consent and Release: Virtual Submissions, the released information may be subject to re-disclosure and no longer be protected by federal privacy regulations or privacy regulations of California or another U.S. state.
• I may revoke this authorization at any time by notifying ESH in writing to legal@educationsuperhighway.org, but such revocation will not have any effect on any actions taken by ESH prior to ESH receiving the revocation.
